Failure mode is the manner whereby the failure is observed. Generally, it describes the way in which the failure happens and its impact on equipment operation.
Failure modes must be determined according to the different component performance standards as identified by the schemas and block diagrams. The individual failure modes for each component are postulated according to the formulated performance demands of the system description and the potential failure definitions included in the ground rules. Since it is possible to have several causes for the same failure mode, all the probable and independent causes for each failure mode must be identified and described.
Physical, chemical or other processes that lead to failure.
«To assist you in prioritizing your improvement efforts, we help you identify the different failure modes and mechanisms and elaborate the hierarchical structures between the failure modes.»
ATTENTION :
It is necessary to distinguish between failure modes and failure mechanisms. Wear and fatigue are not failure modes, they are failure mechanisms.
